How do I roster my team?

Use these links to get all the information and extra forms you need to get your team rostered.

Team Registration Information from the MAHA District 4 Registrar
Each team must have:
1. A roster form signed by player with their name as it appears on their birth certificate. Coaches and managers initials. After you have all your signatures you will need to make 3 additional copies to give to the registrar (total of 4 copies).
2. A USA Hockey IMR (Individual Membership Application) Confirmation. Managers are free. Register at USA Hockey.com.
3. MAHA Background Screening Online Confirmation Page for all coaches, assistant coaches, managers. Fill out online form at MAHA.org and either print or email your confirmation and give to your manager.
4. Signed USA Hockey Coaches Code of Ethics Agreement for each coach.
5. Signed USA Hockey Consent to Treat (Medical Release Form) for everyone on the roster, including coaches and manager.
6. Participant Code of Conduct Agreement for each player on your team.
7. S.T.A.R. Parent Code of Conduct for each player's parent/guardian.
8. Birth Certificate for each player.
Collect all paperwork and group alphabetically by player in this order: Birth Certificate, USA Hockey IMR Confirmation, USA Hockey Consent to Treat, USA Hockey Player Code of Conduct, STAR Code of Conduct.
For coaches, arrange paperwork in this order: USA Hockey IMR confirmation, USA Hockey Consent to Treat, MAHA background screening confirmation, Coaches Code of Ethics.
Drop paperwork at District 4 registrar's house. Allow at least 48 hours turnaround time. If you wish to be notified, leave your email address.
